Your case manager and program outreach specialist can help you manage your complex medical condition.

We'll help you create an individualized care plan with mutually identified goals to meet your acute and long term healthcare needs. We'll provide education to help you understand and manage your medical condition with resources to support your recovery process.

Our team can help you:

• Navigate the complex health care system
• Understand your current medical conditions
• Manage your medication including how to communicate with providers to get the best results from your medication
• Understand how to use available health care services to receive the right care at the right time in the right place
• Identify self-care needs, including arranging referrals to therapeutic services and community-based support resources
• Help you throughout your time as one of our members
